Culture: What is the species like culturally? Are they more war-like? Maybe a peaceful culture? Are the xenophobic as a culture? Is it polite to be more straightforward, or beat around the bush? What is their governmental system,if there is one? etc.
Technology: How advanced are they, technologically speaking? Are they steam powered? Do they have televisions? What about AI''s? Are the medically advanced, and have a cure for most of their native deadly diseases? etc.
History: What is the history of this species? What are their achievements, what wars have they been in, who have the conquered, have they reached interstellar travel? etc.
Present: How is the species presently doing

Number: How many are on their home world presently and how many are somewhere else? Estimates please

Other: What else do we need to know before having this species accepted?

Size: This can be done relatively. I.e. It is X times the size of Jupiter or something
Geography: What is its geography like? Is it all under water? Is the entirety of it mountainous? Are there super volcanoes or miasma bogs? Do they have oceans of methanol? Are they blessed with jungles and coniferous forests, or damned with an eternal desert? Or do they have a combination of all of these?
Climate: Is it like Earths, where it differs on certain parts? Is it always sunny except for one month? Is it usually hot/cold/wet/moist/humid/etc?

Major events/places of high interest: What has happened on the planet that makes it stand out among the rest, and what is something there that will make people go "Oh, isn't that interesting
Main species occupying the planet: Either a canon Doctor Who species, or something of the accepted species list.

Other: Anything else we need to know about your planet before it gets accepted?
